Client Authorization &amp; Minimum Necessary Rule<\/b><\/h4><p data-start=\"1774\" data-end=\"2034\">We obtain <strong data-start=\"1784\" data-end=\"1809\">written authorization<\/strong> from you before requesting medical records. Once received, we share only the <strong data-start=\"1887\" data-end=\"1908\">minimum necessary<\/strong> information with insurers, experts, or courts to prosecute your claim, consistent with HIPAA\u2019s \u201cminimum necessary\u201d principle.<\/p><h2 data-start=\"2041\" data-end=\"2077\">\u00a0<\/h2><h4><b>3. Data Retention &amp; Destruction<\/b><\/h4><ul data-start=\"2079\" data-end=\"2393\"><li data-start=\"2079\" data-end=\"2144\"><p data-start=\"2081\" data-end=\"2144\"><strong data-start=\"2081\" data-end=\"2098\">Active Cases:<\/strong> PHI is retained in encrypted cloud folders.<\/p><\/li><li data-start=\"2145\" data-end=\"2267\"><p data-start=\"2147\" data-end=\"2267\"><strong data-start=\"2147\" data-end=\"2164\">Closed Cases:<\/strong> Medical records are archived for <strong data-start=\"2198\" data-end=\"2209\">6 years<\/strong> (Florida Bar Rule 5-1.2) unless you instruct otherwise.<\/p><\/li><li data-start=\"2268\" data-end=\"2393\"><p data-start=\"2270\" data-end=\"2393\"><strong data-start=\"2270\" data-end=\"2290\">Secure Disposal:<\/strong> Paper files are cross-cut shredded; digital files are permanently deleted or cryptographically purged.<\/p><\/li><\/ul><h2 data-start=\"2400\" data-end=\"2435\">\u00a0<\/h2><h4><b>4. Client Rights Regarding PHI<\/b><\/h4><p data-start=\"2437\" data-end=\"2460\">You may, at any time:<\/p><ol data-start=\"2461\" data-end=\"2638\"><li data-start=\"2461\" data-end=\"2518\"><p data-start=\"2464\" data-end=\"2518\">Request a copy of medical records in our possession.<\/p><\/li><li data-start=\"2519\" data-end=\"2571\"><p data-start=\"2522\" data-end=\"2571\">Direct us to transmit records to another party.<\/p><\/li><li data-start=\"2572\" data-end=\"2638\"><p data-start=\"2575\" data-end=\"2638\">Ask that we delete records we are not legally required to keep.<\/p><\/li><\/ol><p data-start=\"2640\" data-end=\"2733\">To exercise these rights\u00a0call +1 407-929-9292.<\/p><h4><b>\u00a0<\/b><\/h4><h4><b>5.
